Titan Athletics Guide

Athletic Participation Packet

At North Tampa Christian Academy, we are committed to providing a safe, organized, and Christ-centered athletic experience for all of our student-athletes.

Before participating in any practices, tryouts, or games, each student must complete and submit the required participation forms. These requirements are in place to ensure the safety, health, and eligibility of all athletes representing the Titans.

The NTCA Athletic Participation Packet includes:

Athletic Consent and Release Form
Physical Examination Form (valid for 12 months)
Proof of Insurance
Emergency Contact Information
Code of Conduct Agreement

All documents must be submitted and approved by the Athletic Department before a student may participate in any athletic activities.
